Generally, you may set up a trip up to three (3) months before your healthcare appointment. Please set up your ride as many days before your healthcare appointment as possible. To schedule a ride, please call your health plan’s dedicated transportation toll-free number.
When you contact us to schedule a trip, have these things ready:
- Your Medicaid or Member ID number
- Your first and last name
- Your home address including ZIP code for the pick-up
- The name, address, and ZIP code for the healthcare provider you’ll see
- The date and time of your healthcare appointment
Generally, you will need to be ready an hour before your scheduled healthcare appointment. Keep an eye out for your driver.
